NDIS Templates: What Each Document Must Contain

Auditors ask for the same documents again and again. This library explains seven of them: what each one is for, the Practice Standards outcome it supports, and the gaps that cause audit findings.

How this library works

Each page describes one document in plain words. It tells you who needs it, links the Commission's wording for the outcome it provides evidence for, lists what a good version contains, and warns about the mistakes we see most often. The pages explain documents. They do not publish them, and nothing here is a download.

The documents themselves are part of our complete package of 220+ editable Word documents covering the Core Module and Modules 1 to 5, a one-time $1,500 AUD, emailed after purchase. See what is included and pricing.

Where to start

  • Preparing for a first audit. Begin with the incident report form, the participant risk assessment and the service agreement. Auditors sample these from participant files, so gaps show quickly.
  • Delivering support coordination. Read the conflict of interest policy page first. It is the area auditors press hardest for coordinators.
  • Delivering personal care or clinical supports. Start with medication management and the risk assessment, then the emergency and disaster plan.
  • Already registered and facing a mid-term audit. Check your continuous improvement register and emergency and disaster plan, because both show whether your system is still alive.

What auditors look for in every document

Whatever the document, an auditor asks the same four questions. Does it match what you do? Can a worker describe it? Does the participant understand it? And is there a record that it has been used? A document that answers yes to all four is worth far more than a polished one that answers none. Each page in the library lists the checks that matter for its document.

The pages are grouped by the part of the NDIS Practice Standards that each document supports. If you are new to registration, start with our guide to policies and procedures for the full set, then use these pages to go deep on individual documents.

Using templates well

A template saves you from a blank page. It does not do the thinking for you. The NDIS Commission expects the documents you submit to describe how your own organisation delivers supports, and says that copied responses can lead to a refused application or an extra certification audit. Our guide to using NDIS policy templates shows how to adapt any document to your real practice, and how auditors test that you did.

Not sure whether to buy templates or hire help? Read templates, consultants and doing it yourself for an honest comparison, including when a consultant is the better choice.

Other documents you will need

These seven pages cover some of the documents auditors ask for most. The full list, including complaints handling, worker screening records, training registers and the module-specific documents, is in our audit preparation checklist. If your supports include a specific provider type or module, its page shows what the audit adds.
